You are from a low-income family and you need assistance for the beginning of the academic year?

The Fondation maman Dion encourages children from underprivileged backgrounds, from all regions of Quebec, enrolled in elementary and secondary schools recognized by Quebec’s Ministry of Education, to flourish within the school environment, and boosts their self-confidence and their desire to succeed by providing them with essential school supplies, glasses if prescribed by an optometrist, and clothing at the beginning of the academic year.

 

Please read the terms of eligibility in order to check if you qualify for aid.

TERMS OF ELIGIBILITY

1. You must be a low-income family and have permanent resident status in Canada or be a Canadian Citizen;

2. Reside in Quebec;

3. Your child must be enrolled in regular class, in a public elementary or secondary education institution recognized by the Ministry of Education, Recreation and Sports of Quebec (MELS);

4. If your application is accepted, you will need to send us the following :

a) Documentation confirming your total family income as indicated on line 150 of your Notice of Assessment from the Canada Revenue Agency;

b) The first page of the report card of each of your eligible children.

5. If you are able to identify a social worker or any other professional who can confirm your situation, your application may be prioritized;

6. By filling out an online application form, you accept that the Foundation verify certain information about your eligibility;

6. The Fondation maman Dion Selection Committee reserves the right to cancel any application that does not comply;

7. Any application received after 5 p.m., March 27, 2015, will be refused.

 

If you are eligible, please take the following steps to fill in your on line application form for the 2015-2016 academic year.


1. You must complete one form for each child;

2. All forms must be completed online at https://www.jedonneenligne.org/fondationmamandion/ and be submitted by the latest, 5 p.m., March 27, 2015. Please refer to your child’s latest report card and your 2013 Notice of Assessment from Canada Revenue Agency. The maximum time allowed to fill out an online application form is 60 minutes;

3. You will receive the Foundation’s reply, by e-mail, no later than May 29, 2015;

4. Although the Fondation maman Dion would like to help all families, it has unfortunately set limits for some applications because of their number and diversity;

5. Please note that this application will be treated with confidentiality.
